Dear Patients, Friends, and Fellow Professionals,
It was always my plan to put in a full career as an independent family practitioner and then retire to spend more time with my large family and my wonderful wife. It is with great joy that I announce my retirement date of January 1, 2027. I will continue to see patients through the last day of December 2026. Patients will be provided with more than adequate renewals of all current medications to ensure continuity of care during this transition.
It has been an honor to be your family physician. I have been truly blessed to practice family medicine, as it has allowed me to build great relationships, provide essential and preventive health care to so many, and continue challenging myself each and every day. We have planned for a smooth transition for all existing patients. To find a new physician, you can contact your health insurance provider or major local health systems such as Chesapeake General Hospital, Bon Secours, Sentara, and others.
Many of you know that I have been looking for a family physician to replace me for the past three years. Unfortunately, there are very few physicians interested in owning and operating an independent family practice, as the work is demanding, the hours are long, and reimbursements continue to shrink. I strongly recommend that you start looking for a new family physician NOW, as there is a shortage of primary care physicians in the Tidewater area. Beginning this process early will help ensure a smooth transition and minimize any interruption in your care. Your medical records will be available through your patient portal, or in hard copy version for those patients who are not registered on our secure portal, Medent Mobile. If you need assistance accessing your records, please feel free to contact the office. Additional information and alerts regarding medical-record retrieval will also be posted on our website as we begin the transition process with our Electronic Health Records company. Dr. Michelle Paulsen will continue practicing in the Tidewater area and is making plans to relocate to a new facility by January 1, 2027. Once her new location has been finalized, she will communicate that information to her patients.
After 35 years of caring for patients and their families, I want to express my sincere gratitude for the trust you have placed in me. It has been an incredible privilege to serve as your physician, and I will always be grateful for the relationships, experiences, and memories that have made these years so meaningful. Remember to stay healthy and keep your family close.

Records can be requested in the following ways:
1. Message sent via patient portal account, Medent Mobile
2. Transfer to new PCP office; the new office can submit a fax request for patient records and our office will fax records directly to the new PCP. Our fax number is 757-842-6605.
3. Submitting a records request on your own behalf using the following form. Please email form to frontdesk@tfmcpc.com, mail to our office address, or drop off to our front office.
